("Donor") wishes to contribute (a description of the specific bequest or contribution) to the Chariton County Community Foundation ("the Foundation"), a Missouri nonprofit corporation. The Foundation is authorized to accept additional contributions to this Fund.

It is agreed that the contribution will constitute an irrevocable gift to the Foundation for the creation of this fund to be used exclusively for charitable purposes. This contribution shall be administered as a separate fund named the Fund ("the Fund") and shall be part of the Chariton County Community Foundation. The Donor’s contribution and any additional contributions thereto (defined above as the "Fund") may be commingled for investment purposes with other investments of the Foundation and shall be invested in accordance with the rules and procedures of the Foundation. An annual fee of 1%, levied quarterly, will be charged against the Fund’s balances.

The objective of this Fund and the grants to be made from this Fund are to meet the following specific charitable purpose(s) (describe as specifically as possible the purpose(s) of the Fund):The Donor understands that the Foundation, through the Board of Directors ("Directors"), has the right to make grants from the Fund to accomplish the philanthropic intent of the Donor. The organization receiving the grant must be approved by the Internal Revenue Service as a designated charitable agency. It is the Donor’s wish that __% of the assets (or a specific dollar amount of the assets) of the Fund be granted to nonprofit agencies on an annual basis.

All provisions of the Articles of Incorporation, Bylaws and other rules, regulations, policies and procedures of the Foundation which are now in effect and which may be adopted or amended, or both, hereafter shall apply to the Fund.

Receipt and acceptance by the Donor and the Foundation are hereby recognized.
